This is the Mega Drive version of Dragon Slayer: The Legend of Heroes II.
It's the 6th game in the Dragon Slayer series and the 2nd Legend of Heroes game. It ended up being the last game in the series under the Sega Falcom brand. The disappointing result was due to the high expectations it initially had.
Regarding the condition, the manual has scratches, folds, and yellowing. The cartridge is in good condition.
I have confirmed it boots up and have disinfected it with alcohol.
